What this notice covers
This notice describes how Laramie Physicians Women & Wellness Clinic (LPWWC) handles information collected through this website. It does not describe how the clinic handles medical records created during care; those are governed by the clinic's Notice of Privacy Practices provided at the practice.
What the website collects
When you submit an appointment or callback request, the website transmits only the following to the clinic:
- your first and last name
- the phone number and/or email address you provide
- your preferred contact method
- non-clinical scheduling preferences (location, general timeframe, preferred days and time of day)
- your consent selections, along with the exact consent wording and version shown to you
What the website does not collect
Any service or interest selection you make while browsing the request flow stays in your browser. It is not transmitted, stored, logged, emailed or recorded in analytics with your identity. The website does not collect symptoms, diagnoses, medications, medical history or free-text health descriptions, and it does not present a health free-text field.
How the information is used
Submitted details are used to contact you about your request, to route the request to the correct clinic team, and to keep a record that your consent was obtained. Marketing consent is separate, optional, and never a condition of care.
Service providers
The website is hosted and its database and authentication services are provided by the clinic's platform vendor. Transactional email notifications to clinic staff are sent through the clinic's email delivery provider. Bot protection is provided by the site's challenge provider. No advertising, session-replay, chat or marketing vendor is active on this site.
Analytics
Analytics vendors are disabled by default and are blocked entirely on the appointment and callback routes. No analytics event carries your identity or a service selection.
Retention
Requests are retained for the period needed to respond and to maintain a record of consent, then deleted according to the clinic's retention schedule. You may ask the clinic to delete your request record at any time.
Your choices
You may ask what request information the clinic holds about you, ask for it to be corrected or deleted, and withdraw marketing consent at any time by replying STOP to a text, using the unsubscribe link in an email, or contacting the clinic. Withdrawing marketing consent does not affect your care or your appointment request.
