Straightforward answers

Before You Get Started

Who is this program for?

First responders, military personnel, veterans and their referred friends or family can explore the program. Eligibility and supported care location require review; no agency endorsement is implied.

Is the $49 review a membership?

No. It is the initial provider laboratory review. No monthly treatment membership is purchased at the outset.

Can I use insurance for bloodwork?

Where the laboratory and your plan support it, you can request insurance billing. The $49 provider review remains due; deductibles, copays and noncoverage are possible. Coverage is not guaranteed.

What does the $99 cash panel include?

The clinic must confirm the exact included analytes and supported draw locations before online purchase. Additional tests may be required and are not automatically included.

Can I use recent lab results?

Your clinician determines whether outside results meet the required workup and whether more testing is needed. Selecting recent results does not automatically waive bloodwork or the review.

When does treatment billing begin?

Only after required evaluation, an authorized clinician decision, your deliberate treatment choice and a separate verified payment authorization. No treatment is approved by this page or questionnaire.

How are memberships billed?

Annual membership. Convenient monthly payments. Prescription Management is $39/month and Medication Included is $99/month, each billed monthly under a 12-month membership. Annual contract totals are $468 and $1,188, respectively—not upfront charges. LPWWC Testosterone Pellets is separate at $299 every 90 days; four scheduled treatment intervals total $1,196 over 12 months. The initial $49 review and optional $99 cash laboratory panel are separate.

Are Xyosted or Kyzatrex included?

Xyosted and Kyzatrex are available exclusively through the $39/month Prescription Management membership, which covers provider management and prescription coordination. Patients purchase medication separately through the applicable pharmacy or fulfillment provider. The $99/month Medication Included program is limited to injectable testosterone, testosterone cream and enclomiphene; it does not include Xyosted or Kyzatrex.

Does the $99 membership include bloodwork?

No. Eligible medication and standard applicable fulfillment may be included, but bloodwork, the initial review and optional office administration are separate.

Do I bring my own vial for clinic injections?

No. LPWWC stocks medication for approved nurse-administered injections in this program. Administration visits are separately billed; cash fees and eligible insurance arrangements must be confirmed.

Can medication be shipped every 90 days?

Quarterly fulfillment is intended where prescription quantity, pharmacy rules and product dating support it. A 90-day supply is not guaranteed for every formulation.

Where can I receive pellets?

At LPWWC in Laramie only. The $299 per 90-day pathway remains unavailable for purchase until exact medication and procedure inclusions are confirmed.

Will everyone qualify for treatment?

No. Review may lead to additional testing, another evaluation, referral or no medication. Symptoms or a single laboratory value do not guarantee a prescription.

What is shared with my referral contact?

This experience does not disclose your answers or treatment status to a referral contact or employer. Do not enter employer rosters or another person’s health details.

What are cancellation and refund terms?

The clinic must confirm the applicable evaluation and treatment terms before online payment is enabled. No cancellation or refund policy is invented here.
